The common approaches work by changing how the vocal folds vibrate, usually by shortening or increasing the tension of the vibrating part, so that the voice settles at a higher baseline pitch.
It is worth being clear about what surgery does and does not do. Pitch is only one part of how a voice is perceived as feminine; resonance, intonation and the weight of the voice matter at least as much, and these are shaped largely through voice training rather than surgery.
